Summer Sports Physical Dates
Sports physicals will be offered once a month at the Daviess Co. Health Department office. Dates include June 20, July 11, and August 15. All times will be 8:30 to Noon. Reservations are required by calling 660-367-2414 to schedule an appointment. Students must be 6 to 18 years old and accompanied by a parent. Cost is $15 and Medicaid will be billed if applicable. Bring shot record and insurance cards. Amy Heldenbrand, FNP, will be providing the service.
